Summer Camp Grant Proposal Form

• Proposals must be received in our Rockville, MD office by 5:00 pm on June 7, 2010 for full consideration. Proposals received after that date will be reviewed as funds are available. Feel free to contact our office to see if funding is still available.

Acknowledgement of Obligation Form

• If your organization is awarded a summer camp grant an Acknowledgement of Obligation Form must be signed before funds will be released.
• If the recipient fails to meet their obligation, future funding will not be granted.

Release of Information/Photo Release Form

• A release of information/photo release form must be signed for each participant.
• When sending in your reports please only provide information/photos of participants who have permission.
